Together, community health assessments and planning are important tools to use in determining the overall health of a community and identifying key health issues facing our community. The quality of the neighborhoods where we live, work, and play is just as crucial to achieving good health as going to the doctor for regular checkups, proper nutrition, and adequate physical activity.

In collaboration with local hospitals and other community partners, Kane County Health Department is currently in the process of completing an updated Community Health Improvement Plan based on the 2023-2024 Community Health Assessment. Mobilizing for Action through Planning and Partnerships (MAPP) is a framework KCHD uses for conducting the Community Health Assessment. It emphasizes a community-based approach to identify, prioritize, and address current public health issues.

Planning

Our Planning section works closely with the Kane County Division of Transportation and the Development and Community Services Department as part of the Kane County Planning Cooperative. The Cooperative is the integrated effort to implement the County’s array of long-range plans including the 2021-2024 Community Health Improvement Plan, the 2040 Long Range Transportation Plan and the 2040 Plan. Adopted in 2012, the long-range comprehensive Kane County 2040 Plan is a statement of public policy on issues relating to community development, water resource management, open space protection, land use decisions and for the first time, a stand-alone chapter on community health. Healthy People, Healthy Living, Healthy Communities emphasizes the connection between the most important resource in Kane County – its people – with both the opportunities and barriers for healthy living created by our built and natural environment and how they shape the overall health of communities.
