Individuals that are 5 to 17 years of age can only receive the Pfizer vaccine
and must have a parent or guardian's signature on the Pfizer consent form​.

Need a Replacement Vaccination Card?

Kane County Health Department can provide replacement cards to persons who received the COVID vaccine at one of our clinic sites. 
Call 630-208-3801 between 8:30 a.m. and 4:30 p.m. Monday - Friday.

he replacement card can be picked up or mailed. If you did not receive a vaccine from Kane County Health Department, pleas​e go to https://idphportal.illinois.gov​ to request a replacement card from the Illinois Department of Public Health.​​
