Kane County Health Department can provide replacement cards to persons who received the COVID vaccine at one of our clinic sites. Call 630-208-3801 between 8:30 a.m. and 4:30 p.m. Monday - Friday.The replacement card can be picked up or mailed. If you did not receive a vaccine from Kane County Health Department, please go to https://idphportal.illinois.gov to request a replacement card from the Illinois Department of Public Health.
